But the quality is up. Oxy has mostly replaced heroin.

Well that isn't very scientific. War zone type stress. But that is only temporary. What you need for long term PTSD for most of the population is that war zone type stress. We saw that in 'Nam. Once the stress was eliminated only about 20% of the population had long term PTSD (everybody can get it short term). All these years D and you still know almost nothing about PTSD. Now also look at childhood sexual molestation and heroin use. A very interesting subject. PTSD and genetics.
BTW all the heroin available in 'stan at low prices and still only 2.8% of the population are regular users. About double the US. Of course beer might be better. Or worse.
Study the US methadone program origins and results. It is possible you might learn something. Doubtful, but possible. You might also want to learn about who was buying the opium from the Hmong. And the history of opium in Manchuria. And Chiang Kai-shek and opium. Also Japan opium. It is some rather interesting history. Try also Tojo and opium. Vietnam and opium is also a rewarding avenue of research.
The go to guy is McCoy's "The Politics of Heroin" - everyone who knows the field says he is the go to guy. You have read McCoy haven't you? It is free on the 'net. At least an early edition is.
